The Book of Discipline of The United Methodist Church. This package of 32 Official Certificates makes this task easy for both the sending church and the receiving church. The certificate features the image of the cross and flame in a circle; each certificate with a perforated record stub to record this baptism in the permanent church register. The certificate is 8 .5" x 11”, and available in shrink-wrapped packages of 32.
Section 1: The sending church fills out this portion of the certificate and it is kept for the church's records
Section 2: The sending church completes this portion and sends it to the transferring member(s)
Section 3: The sending church completes this portion and sends it to the receiving church (Only this section has space on the reverse side for listing all children transferring with space for the date of baptism if applicable.)
Section 4: The receiving church fills out this portion and returns it to the sending church
